NYC rules require covered RPZ and double-check valve assemblies to be tested at least annually. After the initial report, the next annual report is due within 12 months, and each later annual report is due within 12 months of the previous report.
No single label answers that question. DEP lists many property and business types that generally require devices unless specifically exempted, while the actual requirement depends on the water-service configuration and potential or actual cross-connection hazard. DEP, or a qualified PE or RA where appropriate, should confirm the requirement for a specific property.
Under NYC rules, testing must be performed by a backflow-prevention-device tester certified by the New York State Department of Health and employed by a Licensed Master Plumber. Annual test reports require the Licensed Master Plumber’s seal.
NYC rules say a report showing that the device is not working properly will not be accepted. The defect must be repaired within 30 days and followed by a retest; retest results must be submitted within 30 days after the repair is completed.
The demonstration intake includes new installations so the request can be routed correctly. In NYC, a new installation typically involves a PE or RA preparing the plan, DEP approval and a Licensed Master Plumber performing the installation, followed by certified testing and the required sign-offs.
A DEP notice, prior GEN215-B test form or basic device information is useful. If none is available, send the property address, property type, your best estimate of the device count and any date shown on your notice.
